What are dermal fillers?

Dermal fillers are soft gel injections. They can add volume, shape and support to areas of the face. Most modern fillers are made from hyaluronic acid. This is a sugar-like substance your body already makes. It holds water and helps keep skin plump and hydrated.

Hyaluronic acid fillers are temporary. Your body slowly breaks the gel down, so the effect fades gradually. Some people choose to top up their treatment, and others let it wear off. If needed, this type of filler can usually be dissolved. Your doctor will explain this at your consultation.

Fillers are not the same as anti-wrinkle treatment. Fillers add volume and support. Anti-wrinkle treatment uses a prescription-only medicine that works on the muscles behind expression lines, and it is discussed at consultation. Your doctor can explain which approach suits your concerns, if either.

Would you like a gradual change in skin firmness rather than added volume? Then you may also want to read about Sculptra and polynucleotide treatment.

Lip filler

Lip filler places small amounts of gel into the lips and around the lip border. Our approach is careful and in proportion with the rest of your face. We would rather build up slowly than overdo it.

Lip filler may suit people who:

  • have noticed their lips becoming thinner with age
  • would like a little more shape or definition to the lip border
  • would like more balance between lips that are uneven
  • want to restore some lost hydration and softness

Lips usually swell after treatment, and the swelling is often greatest in the first day or two. Your lips may feel firm or look a little uneven while they settle. This is normal. Many people find the swelling has gone down after about two weeks.

Lips move a lot when you talk and eat, so lip filler tends to fade sooner than filler in other areas. How long it lasts varies from person to person. Your doctor will talk you through what to expect.

Tear trough filler

The tear trough is the groove that runs from the inner corner of the eye down towards the cheek. As we age, we lose some volume here. This can deepen the hollow and cast a shadow that makes you look tired, even when you are not.

Tear trough filler places a small amount of soft gel under the skin to support this area. The skin under the eyes is thin and delicate. This means treatment needs great care and a detailed knowledge of how the face is built.

Tear trough filler does not suit everyone. It tends to work best for people with a clear hollow under the eyes and reasonable skin quality. It is usually less helpful for:

  • dark circles caused by pigment in the skin rather than a hollow
  • puffy under-eye bags or fluid that comes and goes
  • very thin or loose skin under the eyes

If filler is not the right choice, your doctor may talk to you about other options. These could include CO2 laser assisted blepharoplasty for extra eyelid skin.

Cheek filler

Cheek filler places soft gel over the cheek area to add support where volume has been lost. Sometimes supporting the cheek first helps the under-eye area more than treating the tear trough alone. Your doctor will look at your whole face before suggesting where, if anywhere, filler may help.

What happens at your appointment

  1. Consultation. Your doctor will ask about your health, any medicines, allergies and previous treatments. They will listen to what you would like to change and look closely at your face.
  2. Your plan. Your doctor will explain what they recommend, the risks, the cost and the aftercare. You can ask as many questions as you like. If treatment is not right for you, we will tell you honestly.
  3. Treatment. Your doctor will clean your skin before treatment starts. Many fillers contain a local anaesthetic to make treatment more comfortable.
  4. Aftercare. Before you leave, we will give you aftercare advice. We will also tell you how to contact us if you have any concerns.

Recovery and aftercare

Most people go back to their normal day soon after treatment. Swelling, redness, tenderness and bruising are common for a few days. If you have an event coming up, it is wise to plan your treatment well before it.

For the first day or so, your doctor will usually advise you to avoid:

  • make-up on the treated area
  • hard exercise
  • saunas, steam rooms and other strong heat
  • alcohol
  • pressing or rubbing the treated area, unless your doctor has told you to

Risks and side effects

Like any medical treatment, dermal fillers carry some risks. Your doctor will go through these with you before you decide.

Common side effects include swelling, bruising, redness, tenderness and small lumps. These usually settle on their own within days to a couple of weeks.

Less common problems include an uneven result, infection, an allergic reaction and filler moving from where it was placed. If you are prone to cold sores, lip filler can sometimes trigger a flare-up. Under the eyes, filler placed too close to the surface can sometimes give a slight bluish tinge.

Rare but serious problems include filler blocking a blood vessel. This can damage the skin and, very rarely, affect eyesight. This is one reason why it matters who carries out your treatment.

Contact us straight away, or get urgent medical help, if you have severe pain or any change in your vision after treatment. Do the same if your skin turns pale, blue or blotchy.

Who should not have dermal fillers

By law, dermal fillers are only for adults aged 18 and over. Treatment is also usually not advised for:

  • people who are pregnant or breastfeeding
  • people with an active skin infection or cold sore in the area to be treated
  • some people with certain medical conditions or taking certain medicines

Your suitability is always assessed at consultation. If we feel treatment is not right for you at this time, we will explain why and talk about other options.
